Immunization Requirements 

Kindergarten:

5 TdaP, 4 polio, 3 Hepatitis B, 2 MMR and 2 Varicella

7th grade:

Ohio law states that parents must provide proof the Booster Tdap and meningococcal vaccination was given between the ages of 11-13 years of age.  Forms are to be turned into school by the 14th day after school starts. 

These immunizations are required by Section 3313.671 and 3701.13 of the Ohio Revised Code. 

According to state law, we cannot allow your child to attend unless we receive evidence that the above requirements are met by the 14th day after school starts.

 12th grade:

All incoming 12th, grade students are required to show proof of having received the 2 doses of the Meningococcal vaccine by the beginning of the 2018-2019 school year. 1- dose is acceptable if that dose is administered after the age of 16.

Immunizations are required by Section 3313.671 and 3701.13 of the Ohio Revised Code.
