State of Ohio Immunization Requirements for the 2017-2018 School Year

 

Kindergarten

The Ohio Department of Health requires that ALL students entering Kindergarten have the following vaccines; DTap, Polio, MMR, Hep B, and Varicella (Chicken Pox) for the 2017-2018 school year.  Students at Three Rivers Elementary entering into Kindergarten in 2017 will be required to have these immunizations.  Documentation from your child’s healthcare provider must be on file at the school to prevent exclusion.  The reason for this early notification is to provide time for you to schedule an appointment with your child’s healthcare provider to receive the required immunizations and/or obtain documentation of compliance in obtaining these vaccinations.

 

7th Grade

The Ohio Health Department requires that ALL students entering the 7th grade for the 2017-2018 school year has been given the following immunizations; 1 dose of Tdap and 1 dose of Meningococcal (serogroup A, C, W, and Y).  Students at Taylor Middle School entering into 7th grade in 2017 will be required to have these immunizations.  Documentation from your child’s healthcare provider must be on file at the school to prevent exclusion.  The reason for this early notification is to provide time for you to schedule an appointment with your child’s healthcare provider to receive the required immunizations and/or obtain documentation of compliance in obtaining these vaccinations.

 

12th Grade

The Ohio Health Department requires that ALL students entering the 12th grade for the 2017-2018 school year has been given the following immunizations; 2 doses of Meningococcal (serogroup A, C, W, and Y).  Students at Taylor High School entering into 12th grade in 2017 will be required to have these immunizations.  Documentation from your child’s healthcare provider must be on file at the school to prevent exclusion.  The reason for this early notification is to provide time for you to schedule an appointment with your child’s healthcare provider to receive the required immunizations and/or obtain documentation of compliance in obtaining these vaccinations.

 

Students who do not provide documentation of required immunizations are subject to exclusion from school.  To avoid exclusion, we are encouraging you to make an appointment with your healthcare provider to have time to receive immunizations and/or provide documentation of having received immunization(s) before school begins on August 17, 2017.
